Tazpen aka Piperacillin and Tazobactam injection is used to treat bacterial infections in many different parts of the body.

Tazpen aka Piperacillin and Tazobactam combination is an antibiotic that belongs to the group of medicines known as penicillins and beta-lactamase inhibitors. It works by killing the bacteria and preventing their growth. However, Tazpen aka Piperacillin and Tazobactam will not work for colds, flu, or other virus infections.

Each 4.5 g single-dose vial contains Tazpen sodium equivalent to Piperacillin 4 g and equivalent to Tazobactam (Tazpen) 0.5 g and edetate disodium (dihydrate) (EDTA) 1 mg.

Tazpen is an injectable antibacterial combination consisting of the semisynthetic antibiotic Piperacillin sodium and the beta-lactamase inhibitor Tazobactam sodium for intravenous administration. Piperacillin sodium is derived from D(-)-a-aminobenzylpenicillin. The chemical name of Piperacillin sodium is sodium (2S, 5R, 6R)-6-[(R)-2-(4-ethyl-2,3-dioxo-1-piperazinecarboxamido)-2-phenylacetamido] – 3,3 –dimethyl-7-oxo-4-thia-1-azabicyclo[3.2.0] heptane – 2 – carboxylate. Piperacillin sodium is a white, crystalline powder. Piperacillin sodium is freely soluble in water, in alcohol, and in methyl alcohol; practically insoluble in ethyl acetate. Tazobactam sodium is a derivative of the penicillin nucleus. Chemically, Tazobactam is a penicillanic acid sulfone. Its chemical name is sodium (2S-,3S-,5R) – 3 – methyl-7-oxo-3-(1H-1,2,3-triazol-1-ylmethyl)-4-thia-1-azabicyclo[3.2.0]heptane-2-carboxylate,4,4-dioxide. Tazobactam sodium is a white to pale yellow non-hydroscopic crystalline powder.

Tazpen also contains the following excipients: sodium hydrogen carbonate, citric acid (monohydrate), EDTA, water for injections and nitrogen.

Tazpen is indicated for the treatment of the following systemic and/or local bacterial infections in which susceptible organisms have been detected or are suspected:

  • Lower respiratory tract infections;
  • urinary tract infections (complicated and uncomplicated);
  • intra-abdominal infections;
  • skin and skin structure infections;
  • bacterial septicemia.
  • Polymicrobial infections: drug is indicated for polymicrobial infections including those where aerobic and anaerobic organisms are suspected (intra-abdominal, skin and skin structure, lower respiratory tract).

Tazpen, in combination with an aminoglycoside, is indicated for bacterial infections in neutropenic adults or children.

Children Under the Age of 12 Years: In hospitalized children aged 2 to 12 years, drug is indicated for the treatment of intra-abdominal infections including appendicitis complicated by rupture or abscess, peritonitis, and biliary infections. It has not been evaluated in this indication for pediatric patients below the age of 2 years.

Whilst Tazpen is indicated only for the conditions listed above, infections caused by Piperacillin susceptible organisms are also amendable this drug treatment due to its Piperacillin content. Therefore, the treatment of mixed infections caused by this drug susceptible organisms and β-beta lactamase producing organisms susceptible to Tazpen should not require the addition of another antibiotic.

Tazpen is particularly useful in the treatment of mixed infections and in presumptive therapy prior to the availability of the results of sensitivity tests because of its broad spectrum of activity.

Tazpen acts synergistically with aminoglycosides against certain strains of Pseudomonas aeruginosa. Combined therapy has been successful, especially in patients with impaired host defences. Both drugs should be used in full therapeutic doses. As soon as results of culture and susceptibility tests become available, antimicrobial therapy should be adjusted if necessary.


    Tazpen Injection may interact with aminoglycosides (tobramycin, amikacin and gentamicin) which are used to treat bacterial infections. Decreased aminoglycosides blood level has been seen during combined therapy of Tazpen and aminoglycosides given in-vivo to the patients with normal functioning of kidneys, severe kidney disease and having blood purification therapy. When this combined therapy is given in-vitro, piperacillin inactivates aminoglycosides. This antibiotic is not compatible with tobramycin for a single syringe (Y-site intravenous) infusion. Tazpen and aminoglycosides are suggested to be used separately. It is reconstituted, diluted, and used individually when combined therapy with aminoglycosides is given. Tazpen which contains EDTA is compatible with amikacin and gentamicin for Y-site intravenous infusion.
    There may be an interaction of Tazpen with uricosuric-drugs (probenecid), which are used to treat severe pain, redness, and tenderness in joints and excess of uric acid. When probenecid is used in combination with Tazpen, it prolongs the time taken for elimination of half of the Tazpen from the body by blocking the tubular kidney secretion of Tazpen. Probenecid should not be used in combination with Tazpen unless the benefits are more significant than the risk.
    Tazpen Injection interacts with glycopeptide antibiotics (vancomycin) which are used to treat colitis. When vancomycin is used in combination with Tazpen, there is an increased risk of acute kidney injury. It is necessary to monitor the functioning of kidneys in patients having a combination of vancomycin and Tazpen.
    Special instructions need to be followed while taking this medicine along with anticoagulants which are used to prevent or reduce blood coagulation and extend the time of clotting. The combined use of Tazpen with high dose heparin, oral anticoagulants, and other substances may alter the system of blood coagulation including platelet function. It is necessary to perform and monitor the coagulation parameters regularly in patients having the combination of Tazpen with high dose heparin, oral anticoagulants, and other substances.
    Your doctor’s guidelines may need to be followed while taking this medicine along with aminosteroid neuromuscular-blocker (vecuronium), which is used to relax skeletal muscle during surgery. When piperacillin is used in combination with vecuronium, it prolongs nerve and muscle relaxation of vecuronium. Tazpen also shows the same effect if given along with vecuronium. Side effects should be monitor related to nerve and muscle relaxation.
    Tazpen Injection may interact with antimetabolites (methotrexate), which is used to treat cancer, autoimmune diseases and for medical abortions. There is insufficient data available for the combined use of methotrexate and piperacillin which may decrease the clearance of methotrexate. There is no evaluation for the impact of tazobactam on methotrexate elimination. The symptoms of methotrexate toxic effects, as well as blood levels of methotrexate, should be regularly monitored if combined treatment is required.

Before you use, tell your doctor of your medical and health history including the following:
— a genetic disorder affecting lungs and kidneys (cystic fibrosis);
— kidney disease;
— liver disease;
— patients are on a low salt diet;
— sensitivity to multiple allergens;
— sexually transmitted infection (gonorrhea);
— syphilis.
If you are on gonorrhea therapy, you should be tested for syphilis as well. There is an increased risk of rash or fever in patients with cystic fibrosis.
Before you use Tazpen, tell your doctor if you are allergic to it or its ingredients. Your doctor may prescribe an alternative medicine and update your medical records to record this information. Discuss with your doctor if you are allergic to penicillins, cephalosporins, or beta-lactamase inhibitors.
The use of Tazpen Injection may change aspartate aminotransferase (AST) and alanine aminotransferase (ALT). Elevation in the levels of AST and ALT is seen.
The use of this medicine may change alkaline phosphatase. The levels of alkaline phosphatase get increased.
The use of Tazpen Injection may change the bilirubin level. There is an elevation in bilirubin levels.
The use of this medicine may change partial thromboplastin time. There is prolongation in the partial thromboplastin time.
The use of this medicine may change the blood creatinine level. There is an increase in blood creatinine level.

Get emergency medical help if you have signs of an allergic reaction to Tazpen: hives; difficult breathing; swelling of your face, lips, tongue, or throat.
Call your doctor at once if you have:

  • severe stomach pain, diarrhea that is watery or bloody;
  • a seizure (convulsions);
  • low white blood cell countsIclusigfever, swollen gums, painful mouth sores, pain when swallowing, skin sores, cold or flu symptoms, cough, trouble breathing;
  • low potassiumIclusigleg cramps, constipation, irregular heartbeats, fluttering in your chest, increased thirst or urination, numbness or tingling, muscle weakness or limp feeling;
  • signs of inflammation in your bodyIclusigswollen glands, flu symptoms, easy bruising or bleeding, severe tingling or numbness, muscle weakness, upper stomach pain, jaundice (yellowing of the skin or eyes), chest pain, new or worsening cough with fever, trouble breathing; or
  • severe skin reactionIclusigfever, sore throat, swelling in your face or tongue, burning in your eyes, skin pain, followed by a red or purple skin rash that spreads (especially in the face or upper body) and causes blistering and peeling.

Common Tazpen side effects may include:

  • diarrhea, constipation;
  • nausea, vomiting, upset stomach;
  • fever;
  • skin rash or itching;
  • itching or white patches in your mouth or throat;
  • headache, sleep problems (insomnia); or
  • vaginal itching or discharge.

Follow directions on the product label, information guide, and provided by your doctor before using Tazpen. Inject this medicine as per your doctor’s instructions.

The typical adult dose of Tazpen is 3375 mg after every six hours totaled 13500 mg (12000 mg piperacillin/1500 mg tazobactam). The typical dose for children is 100 mg/12.5 mg/kg (45.4/5.69 mg/lb) given after every 8 hours. This medicine is commonly used for a period of 7-14 days for a hospital-acquired lung infection, 5-14 days for a stomach infection, and 5-14 days for urinary tract infection. There are no known habit-forming tendencies with this medicine.
You should continue to use this medicine as directed by the doctor even if you feel well.
The injection form of the medicine may be used on the vein. Inject slowly, into the vein over 20-30 minutes. Each vial of Tazpen reconstituted by adding 50 ml of sterile water or 0.9% sodium chloride solution or 5% glucose for injection under aseptic conditions. Shake vigorously for 1-2 minutes. This solution may be further diluted to the desired volume. Shake well again till it is completely dissolved. Syringes are used for the withdrawn of this solution from the vial. The product should be visually examined for particulate matter, damage to the container or discoloration before use.
